Enrolling at Wagin District High School

Please return the above forms to the school along with your child's:

Birth certificate;
Immunisation History Statement. (This needs to be provided from the Australian Immunisation Register. If you have not been sent a copy, you can access this through your My Gov account.); and
Any other relevant documentation.
